BIRTH CERTIFICATE REGISTRATION (Surat Catatan Kelahiran)

For the Indonesian National who give birth a new born  child  in the Philippines.

·        A Letter  of  request  address to the Head of  Consular Affairs

·       Application form must be completed and signed by the applicant

·        Original and copy  Birth Certificate duly authorized  by NSO

·       Original  and copy of Parent Marriage Certificate  duly authorized by NSO  

·        Original and copy of  Passports

·        Original and copy of Alien Certificate for Registration (ACR) –optional

•   2(two) days processing, not include Saturday, Sunday   and  any Indonesian/Philippines holiday.  
  
•   Administration fee, just look for Consular fee
